We are so excited to meet you. Just a few things to note beforehand so we can make the most of your session when you arrive.

Evaluation

Before Your Appointment

You will get an email from IntakeQ with documentation to complete prior to evaluation, this includes your insurance card for us to assess coverage. If you need to cancel or reschedule your initial appointment, we will miss you, just make sure you give us ample heads up.

What to Bring

Photo ID, insurance card, and referral letter(s) will be gathered on your initial visit. We recommend wearing comfortable clothes that allow easy access for your provider to assess your injury. Shorts, sneakers, and a loose fitting t-shirt is advisable for most issues. In some cases, it is helpful to have your sporting equipment handy (i.e. footwear, etc.). We also encourage people to bring bottled water, as you may be using exercise as part of your plan of care.

Welcome! We can't wait to work together. An evaluation is where we examine the root cause of your symptoms and develop a plan with you to find a path to desired outcomes. We use a combination of hands on manual therapy to address pain, exercise and movement to make you more durable and mobile all while supporting habit development and relationship with movement.

Moving Forward

We make a plan! Based on the information gathered on day one, we will make a recommendation on how much to work together to get desired outcomes. WIth that said, we are a team, so we discuss what fits into your schedule, budget, and life. We work with like minded nutritionists, trainers, physicians, and massage therapists to help support you in this journey.

Our physical therapists offer examination and treatment options consisting of no touch, external touch, and internal assessment based on your comfort level and needs. Our job is to meet you where you are at in order to help achieve your goals. Make sure you communicate with your therapist about how they can best support you.

To support your athletic endeavors it helps if we know about you and your goals. Having a roadmap makes planning and adjusting plans safe, effective, and efficient. An accurate account of your training history, injury history, and medical history will help your provider create a personalized roadmap with you. Afterward you and your provider will discuss key takeaways and strategies to help you reach your goals.

Moving Forward

Subsequent visits will provide progressions specific to your unique needs. If you are attending physical therapy for post-surgical rehabilitation we will contact your surgeon’s office to capture any relevant surgical reports, imaging reports, and post-surgical rehab protocols unique to your surgery.

Nutrition

Before Your Appointment

You will be sent nutrition pre-registration and assessment forms to your email from Healthie. Please fill those out so we are prepared before our appointment to know what your nutrition needs and goals are.

What to Bring

Nutrition appointments are virtual. Check your email to find the appointment link to the online meeting. Ideally a quiet and private location is a great recipe for a productive virtual nutrition coaching session. Schedule here!

At Your Appointment

We love to see our patients, so please use your camera feature on your computer or phone if possible on virtual visits. Also have available any supplements or foods that you have questions on.
